The question
Hello,
Im am 45 . We have male factor infertility and in 2012 we got pregnant on our first try with a non medicated IUI using donor sperm. I had a healthy baby boy via C section. He is 11.5 now. Since then we have done 3 failed IUIs all unmediated . I discovered that I had a blocked tube which was corrected with a Saline sonogram HSG . IN Feb 2023 we did a mini IVF , got 2 eggs at R and only 1 fertilized , graded very good. We did a fresh transfer which failed. In the last 5 months I have lost 55 pds ( was 348 pds , ) corrected my hormones , reversed pr diabetes and have been taking supplements to help with egg quality. I still have a very regular periods that comes at the same time every month and lasts for 4 days. And I ovulate on my own every month. I still get postitive OPK tests at home. These are my labs as of July

the 4 eggs all fertilized with ICSI via the same donor sperm and we froze them at day 3 . I am now waiting for my period to schedule transfer. My question is, how many should I transfer and what protocol would you recommend for my situation. thank you !

You clearly have diminished ovarian reserve. This in conjunction with your age (45y) makes the chance of successful pregnancy using your own eggs, very improbable. You actually need IVF with egg donation. Also, IUI has less than a 1:80 chance of success for you.
If in spite of this information, you absolutely insist on trying with own eggs, you need to be very proactive since the biological clock is on the move and nothing anyone does can reverse this effect.
