I was a patient in 2011. Unfortunately, I lost the baby. I have been having many issues regarding my reproductive organs and I am trying to tie it back to my military service. In 1996, a few weeks after my daughter was born, I was forced to go to advanced training and started hemorrhaging due to physical exertion as I had not healed from the birth. Since then, I have struggled with endometriosis and failure to become pregnant which resulted in my care at your clinic. At the time, we discussed how the trauma from my previous birth caused issues with me getting pregnant in 2011. Since I have been to your clinic, after losing the baby, I had to have a hysterectomy as I was having my cycle 27 out of 30 days and was not able to get pregnant again.
I am hoping to get a copy of my medical records and also ask you to fill out a form stating my challenges and how it was related to previous trauma.
