Hypothalamic amenorrhea

The question

31 F/ G2 T0 P1 A1 L1/5’2.5 103lbs.
Diagnosis: functional hypothalamic amenorrhea. DOR/PCOS/Lactational/thyroid all ruled out.
Background: Past successful pregnancy with letrozol at 114lbs. Have been struggling with HA since then for 12 months. Past OBGYN wanted weight closer to 110lbs, gained to 107lbs and still had no ovulation so letrzol again to at 2.5mg. Conceived. Progesterone high at 37 on 6 days post ovulation. Miscarried due to blighted ovum. Am back down to 103lbs.
Question: Does the hormonal profile of a woman with HA contribute to low egg quality or poor embryo quality? Is my weight too low for safe pregnancy? Also have short cervix. Will low weight make that worse?

Answer from Dr. Geoffrey Sher

Women with HA can experience poorer egg quality. In addition, the low ovarian estrogen output can blunt endometrial response to estrogen. I advise you to have your doctor monitor endometrial thickness at the time of the hCG "trigger". It should measure at least 8mm. If less, this could signal an implantation dysfunction that would require treatment.
